What is a Medical Home?
A Patient-Centered Medical Home Model includes team-based disease management, health coaching, diet/nutrition, counseling, smoking cessation, exercise and close observation. These initiatives will actively engage patients in maintaining their own health, and thereby offer employers and insurance providers a distinctive and innovative choice for their most expensive employees.
What is Chronicare?
Chronicare includes a health risk appraisal that integrates with a physician-supervised personal health management plan and timely health coaching support. Patient compliance with clinical therapeutic regimens is monitored; progress toward health goals is facilitated. Chronicare is designed to motivate people to lead healthier lives and to take a preventive approach to healthcare. Our physicians are partners in this initiative making them invested and committed stakeholders.

Since 1963, the physicians and staff of Jacobs & Modaber have proudly served families in the greater Las Vegas community. With laboratory and radiology services on site, thier goal is to deliver the highest quality medical care in a convenient, comprehensive environment.
At Jacobs & Modaber, the goal is to simplify and hold down the cost of healthcare through a concept called the Patient Centered Medical Home (PCMH). The Physcians at Jacobs & Modaber want to be the foundation of your healthcare—your home. They will be implementing this concept throughout our practice on the coming months.
In 2011 and beyond, Jacobs & Modaber will be expanding their practice to better meet the needs of their patients. As always, they welcome new patients.
